How should I prepare for my first appointment?

Before Your Appointment: You will receive your intake forms, Insurance(if applicable), Medical Consent & Privacy (HIPPA) forms via email. Please fill them out and send them back before your appointment.  

Your Appointment: Please arrive 5 minutes early, well nourished & hydrated, dressed in loose fitting/ comfortable clothing (ie pants or sleeves that can be rolled up over knees & elbows ).

Post Appointment: Ideally you will give yourself the opportunity to rest post-treatment. We live in a busy world, so  this isn’t always possible, but if you can, try and log off for a bit!


Do you take insurance?

We accept out-of-network insurance. Please refer to my insurance page to fill out the insurance coverage form & note that there will be a 2-3 day turn-around time for verification.


What are your rates?

We accept cash, credit card, HSA/ Flex spending at the time of service.

What is your cancellation policy?

Please be so kind as to provide a 24-hour notice to cancel your appointment. Cancellations made after this time period and no-shows will be charged in full.
